1.  To initiate an order with Specialty Optics, please complete our order form (click here: Order Form).

2.  Print out this page and fax it along with your Job Ticket to Specialty Optics: (805) 566-2196.

3.  We will review your order and fax or email you our Payment Authorization Form.  This form will include an Order # that we will assign and the total price, including shipping and handling. 

4.  Once you have received the Payment Authorization Form from us, please complete it with your credit card information, and either fax it Specialty Optics at (805) 566-2196, or telephone this information to:  (805) 745-1690.

5.  Your order will be processed and shipped to you per the method you chose on the initial Order Form.
